This includes all EagleApps modules as outlined below for selection as well as Institutional Awards and Financial Aid (Prosam.) Please note that all requests will be reviewed for approval so it is important to provide detail on why you require this access.

This form is to request EagleApps (student information systems) data for operational reporting / review and analysis purposes. Please provide as much detail regarding your data / reporting needs (type of data, purpose, examples, preferred delivery channel, etc.)
All requests will be reviewed thoroughly by the Student Services Information Systems Data and Reporting team for accuracy, intent, approval to access / view this data and the best source depending upon the requestor's needs / purpose.
