This form is to request EagleApps (student information systems) data for operational reporting / review and analysis purposes. Please provide as much detail regarding your data / reporting needs (type of data, purpose, examples, preferred delivery channel, etc.)
All requests will be reviewed thoroughly by the Student Services Information Systems Data and Reporting team for accuracy, intent, approval to access / view this data and the best source depending upon the requestor's needs / purpose.
This form is to be used to submit application or data issues encountered when using any of the EagleApps Modules including Enrollment, Student Accounts, Institutional Awards or Financial Aid.
